As far as I know. I haven't done this myself yet but you should just be able to go for police clearance and data dot to get it changed onto your name.
The procedure Abri mentions is from scratch basically - introducing it onto the system.
Do yourself a favour and don't change any of the tags and if possible try not to tamper with any of them. If you get one of the staff members who is having a bad day they may make your life difficult accusing you of changing tags of the car (actually illegal in this country as much as many people may not think that is the case).
Your best bet would be to start at the license department and ask them what the procedure is to go. And they should also be able to point you in the direction of the closest police clearance site with the relevant documentation that is required.
